Beacon Health is seeking an enthusiastic individual with a passion for working with adults with severe mental illness and substance use disorders in an evidence-based practice environment. Job responsibilities include, but are not limited to, assertive outreach, knowledge of lake county community resources and linkage, experience or desire to engage SMD and substance abuse population, drive to achieve client centered outcomes and achieve productivity standards, ability to work with multidisciplinary team with a collaborative approach to client care, working knowledge or desire to apply stage wise treatment approaches, harm reduction strategies, and motivational interviewing techniques. Competency in timely documentation and computer skills necessary, and meeting state requirements and standards. Qualifications: Education: Bachelor's degree in social work or related experience and/or education equivalent. Ohio license (LSW, LPC) or CCDC eligibility preferred. Case Manager's are required to pass the QMHS test prior to performing any duties. Experience: Knowledge of mental illness, substance abuse and recovery; experience in a mental health, chemical dependency or social service setting; good interpersonal, verbal and written communication skills essential. General: Must meet satisfactory criteria of the agency with regard to required Bureau of Motor Vehicles driving record check and criminal background check; must have reliable transportation and automobile insurance. Requirements: Satisfactory criminal background check and Bureau of Motor Vehicle driving record; proof of valid driver's license and current automobile liability insurance. Primary Responsibilities: Meets or exceed agency direct service client service standards and goals. Maintains at least 90% compliance with agency Quality Improvement standards. Conduct ongoing assessment of client's mental health needs, substance abuse, and related needs for improving or maintaining successful community living skills. Works collaboratively with clients to develop individual service plans that address client needs for dual disorder services in an integrated manner, incorporate client preferences for services, and work toward client-selected goals, and to monitor the implementation of the plan and progress toward goals. Participates in Dual Diagnosis department and clinical team meetings to further the provision of integrated dual disorders services, and to report/consult with other providers regarding changes in symptoms and mental status. Provides direct service support to clients in their homes and communities, as well as in agency offices, to promote client access to appropriate services. Is knowledgeable of, and makes referrals to appropriate service systems, and serves as a client advocate in navigating various health, education, housing and social service systems to obtain necessary supports. Provides education and support to clients and families regarding crisis management, symptom monitoring, mental illness, substance abuse, relapse prevention, avenues to recovery, and available community supports and self-help services. Secondary Responsibilities Shares on-call responsibilities with other agency staff as needed. Maintains complete and timely documentation of services provided to clients, in accordance with all applicable agency standards and in compliance with all relevant certification, accreditation, and licensure guidelines. Meets or exceeds agency direct client service standards and maintains at least 90% compliance with agency Quality Assurance standards. Participates in agency-wide team meetings, work groups, task forces or committees to promote the improved operation of the agency, as assigned. Interacts in a professional and respectful manner at all times with clients and families, agency co-workers, and other community agency staff. Represents the agency as delegated by the Chief Executive Officer. Participates in annual performance reviews to evaluate that quantity and quality of work meet or exceed agency standards. Essential Functions Comfortable making home visits throughout the community and transporting clients in own car. Able to lift 20-40 pounds. May be required to ascend/descend stairs. Must be able to read, write, speak, and understand English sufficiently to perform duties. Able to reach, kneel, bend and twist at the waist, bend at knees, walk, crouch, see, hear, touch, grasp objects, push, pull, sit. Noise level in work environment is usually moderate. Interested?
