Hospice Center Seeks a Versatile LPN for Fast-Paced INTAKE COORDINATOR Role

Full Time - King of Prussia, Pa

Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks an LPN with strong managerial experience for an Intake Coordinator position. This fast-paced role oversees intake referrals and coordination of hospice services for all patient admissions, and liaises with patients, families, physicians and facilities to ensure the best possible initial care services. Mckesson experience is highly preferred

The Intake Coordinator will have broad-reaching responsibilities and work with a host of departments, medical/non-medical personnel and the agency’s extended client base to ensure outstanding collaboration, support and center integration. Responsibilities include:

  • Receiving case referrals and reviewing available patient information to determine hospice needs
  • Initiating coordination of hospice services in conjunction with the Clinical Coordinator and Director of Clinical Operations
  • Assessing community health needs and aiding in the development of plans/programs to meet those overarching needs
  • Participating in public/community relations activities that promote Grane Hospice’s role as an effective member of the hospice/health care delivery system
  • Working to resolve patient and family concerns/complaints that arise during the referral process
  • Fostering positive relationships with referral sources and the community to promote hospice services in a positive light
  • Assisting with staff and community education programs/projects, as needed

All applicants should be licensed LPNs with previous experience in a hospice/home care setting along with sufficient management/supervisory work. Candidates are expected to have excellent observation, good nursing judgment and sound communication skills, with a professional understanding of hospice philosophy and issues surrounding death/dying. A working knowledge of pain/symptom management is also essential. Previous marketing and McKesson experience is highly preferred.
